Online passport application from the Consulate’s website www.cgisf.org only should be completed for submission to the Consulate General of India, San Francisco
Online applications have been introduced for providing more efficient and faster services by the Consulate. Your cooperation is, therefore, necessary. The system is designed to accept ‘n’ number of applications in one day. When the ‘n’ number of applications has been reached for the given day, the appointment date rolls over to another date. Take care, therefore, in completing the application in one attempt. If you do it more than once, it affects the others. Please do not try to bypass the system as any deviation will not allow you to print the form.
On clicking the online application, different dialogue boxes pop up for different browsers::

The application is in a secured platform using HTTPS (Hypertext Transfer Protocol over Secure Socket Layer) used to indicate a secure HTTP connection

If you do not accept the appointment date , you will not be able to print out the application after completing the entries
- After the date of appointment has been accepted, the following form will appear:

(*) denotes mandatory field
- While completing the online application, please do not enter any special characters such as (,), (.) (/) or symbols such as (@), ($), etc
- Follow instructions given in the online application
- In the box ‘Indian Mission,’ select ‘USA-SAN FRANCISCO’ from the dropdown menu
- The box 'Service Desired' has the following four services
- Passport for Children upto 15 Years of Age
- Fresh Passport
- Re-issue of Passport
- New Passport in Lieu of Damaged/Lost Passport
- Surname is normally the last name. In the ‘Surname’ box, type in your last name e.g. Shah, Das, Chatterjee, etc
- In the box ‘Given name,’ type in your name/s other than the surname. Enter the names as they appear in your passport. The box should contain the names in the order of first name, middle name, and other names.
Note :
- Enter your given names/surname in the same order with the same spelling as they appear in your old passport.
- Initials, if any, should be expanded
- If you have changed your name from the one displayed in your old passport, you should then follow procedure of Change of Name service.
- As your name and surname will appear on the passport, take care to enter the details correctly, avoiding any mistakes.
- In the box ‘Previous Name,’ enter the full name that you were known as earlier. This is applicable to anyone who has even marginally changed his/her name. If there is no change in name, leave the box blank. A married female applicant should enter her full name as it existed before marriage. This is commonly known as ‘maiden name.’
- In the box ‘Date of Birth,’ enter first the date followed by the month and the year in that order.
- In the box ‘Place of Birth,’ enter the name of place like Village; Town; and the State in India. If born outside India, mention the place, state and the country
- Against the column “Address to be printed in passport,” enter your permanent address in India
- Against the column “other address” enter your address in the USA
- In the boxes for father’s, mother’s and spouse’s names, enter the full name with surname mentioned after the given name/s. Enter their nationality.
- Even while you enter your ‘Phone No.,’ ‘email address,’ and ‘mobile’ number’ details in the respective boxes, these are not captured in the print out. It is, therefore, necessary, more so for applicants availing of services by mail, that these details are handwritten on the first page to enable the Consulate contact them if need arises
- If you have held a passport earlier, check the box and enter the previous passport particulars
- The File Number is generally on the last page of the earlier passport
Note: In the process of completing the entries, if you fail to complete the mandatory fields, you will be prompted to complete it. If information in the mandatory fields is not completed, you will not be able to obtain a print out.
Most of the particulars that you enter on line appear in the passport. It is, therefore, imperative that you take care and enter the particulars correctly.

FOR PROCEEDING FURTHER, CLICK ‘CONFIRM THE APPOINTMENT’ ON CONFIRMING THE APPOINTMENT, FOUR (4) PAGES WILL PRINT OUT. COMPLETE THESE PAGES AS EXPLAINED IN THE ILLUSTRATIONS.
- Page one and columns 5, 6, 8 and 10 on page 2 will appear with completed entries; There will be still some columns on pages 2, 3 and 4 which will be required to be completed by hand.
- Complete in BLOCK letters and with black or blue ballpoint pen the rest of the blank entries on pages two, three and four;
Photographs:

Paste (not staple) your photo in the box on first page and on page 4. Two (2) photos of 2 inches x 2 inches or 35mm x 35mm photos – one photo to be pasted on Page 1 (one) in the box and the 2nd photo to be stapled on the blank portion (below ‘for official use only’) on page 4 (four)
- Sufficiently bright colour photo with frontal view of full face against a light background
- Close-up of head and top of shoulders with the face filling up 70% to 80% of the photo
- Sharp focus, clear, and of high quality
- Following kinds of photographs are not accepted
- Black and white photographs
- Photographs with coloured or dark glasses
- With dark background or in uniform
- Polaroid, home-shot or home-scanned, or unduly large/small photographs are not accepted
- Photograph should be recent and in no case more than six months old
- If there is distinct difference between your photograph in the earlier passport and the current photograph, sign on the front of the second photograph on the 'blank part of page 4' and have it notarized (follow process of ‘Change of Appearance
- Signature/Thumbimpression
- Sign in the box provided for signature on the page 1 (one) to the left of the photograph
- Sign below column 26 (DECLARATION)
- Sign in the boxes in Column 29 on page 4
- If applying for ‘Passport for Children upto 15 Years of Age (minors),’ both parents should complete entries in 'column 27 on page 3' and sign or affix thumb impression on page 4. You should then present yourselves to a registered Public Notary for notarizing the signatures
- Those who cannot sign should affix their thumb impression (right thumb for female applicants and left thumb for male) in the boxes provided for signature; (Applicants should either append their signatures or affix their thumb impression and not do both)
- If APPLYING IN-PERSON at the Consulate, present the hard copy of the completed application with all supporting documents at the Consulate ON THE DATE OF APPOINTMENT generated on the extreme right of page 1 of the application. If APPLYING BY MAIL, post the application irrespective of the date of appointment generated on the application.
Note: Note down and use the web file number (which starts with USASI……) generated on the application for corresponding with the Consulate, mentioning full name of applicant.
