New Passport (After 10 years and/or does have an "F" after the expiry date), Change of appearance, Change of name, Lost/Damaged Passport, EC.
i) Change of Name Consequent to Marriage also get form Affidavit Form
ii) Change of Name NOT Consequent to Marriage,  get form Affidavit for Change in Name
iii) Lost/Damaged Passport Applicant, get form Personal Particulars Proforma and Sworn Affidavit

iv) A separate Affidavit is required for passport which have expired for more than 6 months.
