(Important Note)


SOME USEFUL INFORMATION ABOUT YOUR PASSPORT  

i.          Please keep a photocopy of your passport in a safe place. Without these relevant issuance of a duplicate passport in case of loss/damage/theft may be delayed.  

ii.         It is an offence under the Passports Act 1967 to give false information in the Application. Passport facilities can be denied on grounds of suppression of factual information, submission of false particulars, willful damage of passport and for making unauthorized changes in the passport.

iii.       Your passport can be impounded or revoked by the Government of India under the provisions of the Passports Act 1967, in the event of which you will have to surrender the Passport to the nearest Indian Passport authority.  

iv.        Indian Citizenship Law Does Not Provide For Dual Citizenship and, accordingly, it is illegal for a person to possess an Indian Passport once he/she becomes citizen of another country. It may therefore, be noted that on becoming citizen of any other country (including US), the holder of an Indian passport should return the same to the nearest Indian Consular post (Embassy/Consulate). The Indian passport will be stamped "acquired US nationality" and returned to holder. Please send the Indian passport with covering letter and a self addressed stamped envelope for this purpose.

 

v.         Please always check the expiry date of your Passport, and get it renewed well in advance of the expiry. Incase your Passport has expired more than six months back the process of renewal may take longer time.  

 

vi.        Passport should not be sent out of any country by post. It should be in the custody of the holder or especially authorized by the holder. It must not be altered or mutilated in any way. Loss, theft or destruction of Passport should be immediately reported to the nearest passport authority in India or Mission abroad and to the local police authorities.

LIFE CERTIFICATE

What are the documents required for this service? 

i)                    Completed Miscellaneous Services form to be submitted by the applicant in person at the Consulate

ii)                   One Photograph (preferably in colour, front facing, 2" X 2" )

iii)                   Original Indian passport

iv)                 If application is sent by mail, the signatures of the applicant on the application should be notarized.

v)                 In case the Life Certificate has to be submitted in a prescribed Proforma, the original signed by the applicant and a copy thereof

vi)                  There is no fees for this service.

vii)                 A SELF ADDRESSED STAMPED USPS EXPRESS MAIL ENVELOPE WITH APPROPRIATE STAMP SHOULD BE SENT FOR RETURNING PASSPORT/DOCUMENT FOR EACH APPLICANT.

MARRIAGE CERTIFICATE

What are the documents required for this service and how much time will this service take? 

i)                    Completed Miscellaneous Services form 

ii)                   One Photograph (preferably in colour, front facing, 2" X 2" )

iii)                  Original Indian passports of both husband and wife. At least one of the passports should have the entry of spouse’s name. If both the passport do not have the entry regarding name of spouse, please see instructions below. Marriage certificate will be issued only after addition of spouse's name in both the passports.

iv)                 Fees of $ 20

What are the documents required for obtaining entry of spouse's name?

iii)           Original Indian passport of both husband and wife.

iv)             Copy of marriage photographs and marriage invitation card OR Indian marriage certificate (translated, if in a language other than English) Alongwith an affidavit signed and notarised by both parties to the marriage.

v)               Fee of $10 for each entry (total $20 for services on both passports)

iii)                 Since it takes 30-35 days for the issue of PCC, you must submit the photocopy of the passport at the time of applying and present the original at the time of issue of the PCC as it will have to be stamped.

ATTESTATION OF DOCUMENTS/ POWER OF ATTORNEY

a)      A photograph of the Executant has to be affixed on the POA and notarized by the Notary Public.

b)      In case the Notary Public does not notarize the photograph on the POA, it could be pasted on a separate sheet of paper and notarized as an Annexure to the POA. 

vii)            Attestation of Photograph

A fee of $10.

viii)                Fees of $ 20 for property matter and general attestation (each attestation) or $ 50 for attestation of Commercial documents (each attestation). See special instructions below for Commercial documents and Adoption papers.

Does the Consulate provide the stamp papers for making POAs?

No. 

What are the special requirements for attestation of Commercial documents or Adoption documents? 

Documents pertaining to Companies (Commercial documents) and from Adoption Agencies must first be got attested by the Authentication Division of the State Department. Copies of all documents have to submitted for retention in the Consulate.

IMPLEMENTATION OF THE 'APOSTILLE' CONVENTION

With India’s accession to the Hague Convention of October 5, 1961 abolishing the requirement of legalization of foreign public documents (www.hcch.net) Government of India, Ministry of External Affairs at CPV Division, Patiala House will start issuing Apostilles w.e.f. August 29, 2007. Apostille stickers will be issued on the documents –personal like birth, death, affidavits, power of attorneys, marriage etc and educational documents like degree, diplomas, matriculation and secondary level certificates. However, commercial documents are excluded from the purview of the Convention.

Procedure: All documents should be first authenticated by the designated agencies in the State from where thedocument has been issued. In case of personal documents, Home Department/General Administration Department arethe designated authorities. In case of educational documents, State Governments have now opened Regional Authentication Centers (RACs) (see list) at various States from where the document should first be attested. Documents attested by these authorities then need to be presented to MEA for issue of Apostilles.

Fee: A fee of Rs. 50/- in form of Postal/Pay Order drawn in favour of PAO, MEA, New Delhi is payable for each Apostille sticker.

DEATH DOCUMENTS ATTESTATION

What documents are required to be attested for shipment of dead body / cremains to India?

EMERGENCY CERTIFICATES

What are Emergency Certificates?

An Emergency Certificate is a one way Travel Document which authorizes an Indian national to travel to India. It is issued to a citizen of India whose passport has been lost/ damaged/ stolen, and who needs to return to India urgently without waiting to complete the formalities for issue of a duplicate passport. The applicant has to provide complete information about his Indian nationality to the Consular Officer.

What are the documents required for obtaining an Emergency Certificate?

i)                    Application Form For a New Passport , Declaration in the form must be notarized

ii)                   6 photographs (colour, front facing, 2" X 2" ). One photograph  to be pasted on the application and notarized.

iii)                 Personal Particulars Proforma

iv)                 Police Report about the loss of passport

v)                  Copy of lost passport

vi)                 Copies of school certificates/ birth certificate/ Indian driving license / ration card/ Election Commission I.D./ employment letter/ any other photo-identification from India (whichever available)

vii)               Letter from school / college , if student

viii)              Proof of residence in USA from the date indicated in the application such as copies of utility bills and copy of lease deed of house/ flat occupied/ bank certificate/ insurance policy

ix)                 Copy of Green Card/ Employment Authorization Card, as applicable. The Consulate may demand to see the original.

x)                  Fee of $5

xi)                 Two sworn affidavits in the prescribed form from Indian nationals resident in USA, duly notarized, certifying that they know you and that you are an Indian citizen (copies of relevant pages of the passports of the two persons giving the sworn affidavits to be enclosed)

xii)               Applications for issue of EC are neither received nor ECs dispatched by post. The applicant has to be present personally before the Consular Officer while filing application for an EC. In exceptional circumstances only, mail back charges will be accepted. No self addressed envelope necessary. Fees for EC+ mailing charges can be paid together through one Money Order or Cashier’s Check (or in cash at the counter).

EMERGENCY SERVICES

 

22.       For Emergency Services (Visa) additional amount of $35.00 is charged. However the emergency fees/charges for issue of out of turn Indian Passport (only accepted in the Consulate at the counter - not by mail) shall be US $90.00. Such passports shall not be issued on closed holidays. Such services are provided only in cases of genuine emergency such as death or serious illness in the immediate family and not for cases falling in the prior approval category, requiring clearance from India.
