NORI (Letter of Need)

GUIDELINES AND INSTRUCTIONS

· Copy of the written assurance from the candidate in the Prescribed Form. 

· A copy of the contract with the hospital / institute on the standard agreement form.

· A copy of the offer letter from the University / Medical School on the official letter head.

· A copy of the previous No Objection Certificate (NOC) issued by the ministry of Health and Family Welfare (MHFW) and / or Embassy / Consulate.

· Copy of the current DS-2019 Form

· A copy of the bio-data with current address, telephone and fax numbers, email, etc.

· A copy of the passport showing personal particulars

· A US stamped self-addressed envelope.
